Travel vaccinations

If you require vaccinations relating to foreign travel, you MUST first complete a Travel Questionnaire  for each person travelling who is registered with the Practice and return to us a minimum of 10 weeks before your departure date.

When the questionnaire(s) have been assessed you will receive a telephone call from the Practice Nurse advising the vaccinations required for your destination and they will make you an  appointment for a Thursday afternoon.

 

(Please note that some vaccinations incur a fee)
