School Hill Medical Practice will shortly be completing the Summary Care Record upload.
This means that your Summary Care Record will be available to help emergency Doctors and Nurses help you if you need to contact them when the surgery is closed.
Initially, it will contain just your current medications and allergies. Later on as the system develops, more information will be added but patients will be informed when this is due to happen.
This does not mean that people will have unlimited access to your personal information. Staff will still be required to obtain your verbal consent before they are able to view your record. Access to your records is also closely monitored by a range of audits.
If you do not wish to have a Summary Care Record, you must complete an Opt Out form.
Please ask at Reception for one of these and return to the surgery as soon as possible.
